Nature of the submission: Report of Marriage

To Whom It May Concern:

Enclosed please find my Report of Marriage Contracted Abroad form and supporting documents.

Contents include:

–      Four (4) original duly-accomplished Report of Marriage Contracted Abroad form

–     One (1) original or certified true copy and four (4) photocopies of the Marriage Record

–     One (1) original or certified true copy and four (4) photocopies of the Marriage Certificate or Contract

–     Four (4) photocopies of a government-issued identification card of both husband and wife

–     Four (4) photocopies of Philippine birth certificate of Filipino spouse

–     Self-addressed return envelope, with appropriate stamps for express or priority mail with tracking numbers via US Postal Service

–     Processing fee of $25.00, Money order

Copies of documents submitted are exact photocopies of unaltered documents and I understand that I may be required to submit additional proof or documents  to prove my  citizenship or identity pursuant to the Philippine Passport Law at a later date.

  1. Hi Yuri, How you doing?Anyway here’s my question.
    1. In the ROM form at the very top they have it there date of report, foreign service post and ID#, should I fill it out or just leave it blank?
    2. And also do you have a sample on how to write a check that payable to Philippine General Consulate?I look for a sample but I can’t find 1 po kasi. We got married in Vegas and my jurisdiction is Los angeles.
    3. In affidavit for delayed registration of marriage should I also put my maiden name just like in the ROM form?
